Gr\u00e2ce \u00e0 elle, lorsque les cellules trait\u00e9es sont expos\u00e9es \u00e0 une longueur d\u2019onde lumineuse sp\u00e9cifique, elles produisent un courant \u00e9lectrique qui permet de <strong>r\u00e9tablir la communication nerveuse<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Optog\u00e9n\u00e9tique : qu&rsquo;est-ce que c&rsquo;est&nbsp;?<\/h2>\n\n\n\n<p>Chez les \u00eatres vivants dot\u00e9s d\u2019un <strong>syst\u00e8me nerveux<\/strong> (\u00ab eum\u00e9tazoaires \u00bb), celui-ci a deux r\u00f4les principaux. En premier lieu, il intervient au niveau des <strong>sens<\/strong>, comme la vue ou l\u2019ou\u00efe, en transf\u00e9rant des signaux provenant du milieu ext\u00e9rieur au cerveau, qui se charge ensuite de les analyser. Par ailleurs, les <strong>nerfs moteurs<\/strong>, qui vont du syst\u00e8me nerveux central (cerveau et moelle \u00e9pini\u00e8re) aux muscles, permettent de contracter ces derniers pour <strong>d\u00e9clencher des mouvements<\/strong>. <\/p>\n\n\n\n<p>Dans les deux cas, c\u2019est l\u2019\u00e9tablissement d\u2019un<strong> courant \u00e9lectrique<\/strong> qui permet cette communication. <\/p>\n\n\n\n<p>Si les capteurs naturels que sont l\u2019\u0153il et l\u2019oreille sont incapables de traduire les signaux ext\u00e9rieurs en flux \u00e9lectrique ensuite transmis aux nerfs, <strong>le cerveau ne re\u00e7oit pas les informations<\/strong>. Autrement dit, les fonctions visuelles et auditives deviennent inop\u00e9rantes. <\/p>\n\n\n\n<p>De m\u00eame, si un nerf moteur ne peut pas faire parvenir au muscle associ\u00e9 une onde \u00e9lectrique, la contraction musculaire ne se fait pas et les mouvements deviennent impossibles. <\/p>\n\n\n\n<p>L\u2019optog\u00e9n\u00e9tique a pour objectif de<strong> restaurer la possibilit\u00e9 de g\u00e9n\u00e9ration d\u2019un courant \u00e9lectrique par les organes sensoriels ou les nerfs qui n\u2019en sont plus capables<\/strong>. <\/p>\n\n\n\n<p>Pour cela, le principe est d\u2019introduire dans les cellules \u00e0 traiter<strong> un g\u00e8ne d\u00e9clenchant la fabrication d\u2019une prot\u00e9ine \u00ab photosensible \u00bb<\/strong> qui r\u00e9agit \u00e0 une longueur d\u2019onde lumineuse sp\u00e9cifique. <\/p>\n\n\n\n<p>Une fois r\u00e9alis\u00e9e cette op\u00e9ration de <strong>th\u00e9rapie g\u00e9nique<\/strong>, lorsque les cellules trait\u00e9es sont expos\u00e9es \u00e0 cette longueur d\u2019onde sp\u00e9cifique, elles g\u00e9n\u00e8rent un courant \u00e9lectrique. L\u00e0 sont localis\u00e9es des <strong>cellules photor\u00e9ceptrices<\/strong>, les c\u00f4nes et les b\u00e2tonnets, qui ont la capacit\u00e9 de <strong>traduire l\u2019information lumineuse re\u00e7ue en signal \u00e9lectrique<\/strong>. Celui-ci est ensuite transmis au <strong>nerf optique<\/strong>, puis, via ce dernier, au <strong>cerveau<\/strong>. <\/p>\n<\/div><\/div>\n\n\n\n<p>Certaines pathologies oculaires, \u00e0 l\u2019origine de c\u00e9cit\u00e9, ont pour cause un <strong>dysfonctionnement des cellules photor\u00e9ceptrices r\u00e9tiniennes<\/strong>. <\/p>\n\n\n\n<p>C\u2019est notamment le cas de la <strong>r\u00e9tinopathie pigmentaire<\/strong>, dont de r\u00e9cents essais de traitement optog\u00e9n\u00e9tique ont donn\u00e9 des <strong>r\u00e9sultats extr\u00eamement prometteurs <\/strong>bien que la m\u00e9thode en soit encore au stade exp\u00e9rimental. <\/p>\n\n\n\n<p>Notamment, au cours de l\u2019une de ces \u00e9tudes, \u00e0 l\u2019aide d\u2019un vecteur viral (ad\u00e9novirus) classiquement utilis\u00e9 en th\u00e9rapie g\u00e9nique, les chercheurs ont ins\u00e9r\u00e9 dans le g\u00e9nome de certaines cellules r\u00e9tiniennes un g\u00e8ne codant pour la<strong> synth\u00e8se d\u2019une prot\u00e9ine<\/strong> d\u2019origine algale et de la famille des opsines. Cette mol\u00e9cule induit la <strong>production de courant \u00e9lectrique <\/strong>par les cellules modifi\u00e9es g\u00e9n\u00e9tiquement lorsqu\u2019elles sont expos\u00e9es \u00e0 de la<strong> lumi\u00e8re \u00ab ambr\u00e9e \u00bb<\/strong>. <\/p>\n\n\n\n<p>Le patient a alors \u00e9t\u00e9 \u00e9quip\u00e9 de <strong>lunettes<\/strong> particuli\u00e8res, qui convertissent les ondes lumineuses re\u00e7ues par l\u2019\u0153il en images aux teintes rouge-orang\u00e9es d\u2019intensit\u00e9 variable. Sous l\u2019effet de ces stimuli, les<strong> cellules r\u00e9tiniennes transg\u00e9niques<\/strong> sont alors devenues capables de produire un courant \u00e9lectrique ensuite transmis au cerveau par le biais du nerf optique. <\/p>\n\n\n\n<p>Apr\u00e8s plusieurs mois d\u2019apprentissage et d\u2019adaptation, le sujet humain sur lequel a \u00e9t\u00e9 men\u00e9 ce protocole exp\u00e9rimental est aujourd\u2019hui capable de <strong>discerner les objets qui l\u2019entourent<\/strong>. <\/p>\n\n\n\n<p>Ces espoirs de traitement optog\u00e9n\u00e9tique des cas de r\u00e9tinopathie pigmentaire ont depuis \u00e9t\u00e9 confirm\u00e9s par les r\u00e9sultats obtenus au cours d\u2019une autre \u00e9tude sur deux sujets atteints de <strong>c\u00e9cit\u00e9<\/strong> depuis plus de 15 ans. <\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>D\u2019autres essais sont encore en cours et il faudra du temps avant que cette technique ne soit \u00e9ventuellement disponible en routine. Elle offre n\u00e9anmoins de<strong> belles perspectives de traitement pour les sujets atteints de c\u00e9cit\u00e9<\/strong> mais qui pr\u00e9sentent un <strong>nerf optique fonctionnel<\/strong>.<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\">Application de l\u2019optog\u00e9n\u00e9tique \u00e0 la restauration de la fonction auditive<\/h2>\n\n\n\n<div class=\"wp-block-media-text alignwide has-media-on-the-right is-stacked-on-mobile\"><div class=\"wp-block-media-text__content\">\n<p>Lorsque l\u2019<strong>audition<\/strong> est fonctionnelle, les <strong>ondes sonores <\/strong>font vibrer le tympan, qui les transmet \u00e0 un ensemble d\u2019os, les <strong>osselets<\/strong>. Ces <strong>vibrations<\/strong> transitent ensuite jusqu\u2019\u00e0 la cochl\u00e9e, dans l\u2019<strong>oreille interne<\/strong>, au niveau de laquelle elles sont traduites en<strong> signal nerveux <\/strong>et atteignent le <strong>nerf auditif<\/strong>. <\/p>\n\n\n\n<p>Certaines surdit\u00e9s trouvent leur origine dans une <strong>d\u00e9ficience de communication entre la cochl\u00e9e et le nerf en charge de transmettre le signal au cerveau<\/strong>. La premi\u00e8re \u00e9tape a consist\u00e9 \u00e0 introduire le <strong>g\u00e8ne d\u2019une opsine<\/strong> dans le g\u00e9nome de leurs cellules cochl\u00e9aires. Puis, un <strong>implant externe<\/strong> a \u00e9t\u00e9 mis en place, pour <strong>transformer les vibrations sonores en signaux lumineux<\/strong>. Ceux-ci \u00e9taient alors v\u00e9hicul\u00e9s jusqu\u2019\u00e0 la cochl\u00e9e via une<strong> fibre optique<\/strong>. Ainsi, sous l\u2019effet de la lumi\u00e8re transmise, les cellules de la cochl\u00e9e contenant le g\u00e8ne de l\u2019opsine ont permis de<strong> g\u00e9n\u00e9rer un courant \u00e9lectrique <\/strong>pour <strong>stimuler le nerf auditif<\/strong>. <\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>Les r\u00e9sultats de ces essais de laboratoire ont \u00e9t\u00e9 suffisamment prometteurs pour que le <strong>d\u00e9marrage de tests cliniques sur l\u2019Homme<\/strong> ait \u00e9t\u00e9 d\u00e9cid\u00e9 <strong>en 2025<\/strong>.<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\">Optog\u00e9n\u00e9tique et handicaps moteurs<\/h2>\n\n\n\n<p>Certains <strong>handicaps moteurs <\/strong>sont dus \u00e0 une<strong> absence de stimulation nerveuse des muscles<\/strong>. Or, certaines \u00e9tudes r\u00e9centes ont montr\u00e9 qu\u2019il \u00e9tait possible de rendre les <strong>cellules du muscle cardiaque sensibles \u00e0 la lumi\u00e8re<\/strong>, l\u00e0 encore apr\u00e8s introduction dans leur ADN d\u2019un g\u00e8ne d\u00e9clenchant la <strong>fabrication d\u2019une prot\u00e9ine photosensible<\/strong>. <\/p>\n\n\n\n<p>\u00c0 l\u2019origine, l\u2019id\u00e9e \u00e9tait de <strong>perfectionner les dispositifs cardiologiques implantables<\/strong> (pacemaker) pour les rendre moins \u00e9nergivores. Mais, plus g\u00e9n\u00e9ralement, ces r\u00e9sultats sont porteurs d\u2019espoir pour les<strong> patients paralys\u00e9s par atteinte de la moelle \u00e9pini\u00e8re<\/strong>. <\/p>\n\n\n\n<p>En premier lieu, sans parler d&rsquo;optog\u00e9n\u00e9tique, l\u2019<strong>\u00e9lectrostimulation<\/strong> a donn\u00e9 ces derni\u00e8res ann\u00e9es des r\u00e9sultats particuli\u00e8rement satisfaisants. Ainsi, des patients parapl\u00e9giques \u00e9quip\u00e9s d\u2019un bo\u00eetier de commande dispensant des <strong>impulsions \u00e9lectriques au niveau de la moelle \u00e9pini\u00e8re<\/strong> sont redevenus <strong>capables de bouger<\/strong> leurs membres inf\u00e9rieurs. Certes, leur marche n\u2019est pas fluide et n\u00e9cessite, apr\u00e8s un long entra\u00eenement, l\u2019utilisation d\u2019un d\u00e9ambulateur. <\/p>\n\n\n\n<p>Il s\u2019agit quoi qu\u2019il en soit d\u2019une avanc\u00e9e extraordinaire. L\u2019\u00e9tape suivante sera certainement de mettre en place des<strong> traitements exp\u00e9rimentaux bas\u00e9s sur l\u2019optog\u00e9n\u00e9tique<\/strong>. <\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>Des premiers tests optog\u00e9n\u00e9tiques ont d\u00e9j\u00e0 \u00e9t\u00e9 r\u00e9alis\u00e9s en laboratoire sur des souris paralys\u00e9es.
